What to know before you arrive at the Passport Office

  • If you need your passport in two weeks or less, please call the National Passport Information Center ( 1-877-487-2778 ) to make an appointment at the Arkansas Passport Center in Hot Springs in order to get your passport processed in time.
  • Mondays, Wednesdays, and Thursdays are for scheduled appointments only.
  • Tuesdays are the only days we take walk-ins.
  • All applicants must appear in person to have an application processed.
  • Applicants under the age of 16 must have both parents or guardians present.
  • If you are applying through the library (we send the application), we will take your photos at no additional cost to you. If you only need photos, there will be a $5.00 charge.
  • We do not process U.S. Passport renewal applications, only new applications. Scroll to the end of this page for information regarding renewal applications.

NEW Passport Applications: What is needed to apply

  1. Completed application forms - Forms are available at the library or online at www.travel.state.gov
  2. Social Security Number
  3. Proof of citizenship – at least one of the following items
    • Certified copy of United States Birth Certificate with a raised or colored seal (photocopies, scanned computer prints or hospital birth records are not acceptable)
    • U.S. Citizen’s Birth Abroad papers
    • Previous United States Passport
    • Naturalization papers
  4. Current photo identification – one of the following
    • Driver’s license
    • State identification card
    • Military identification
    • Previous passport
    • For minors (applicants under 16), both parents must appear with photo IDs. If only one parent is present, the absent parent must provide a completed notarized DS-3053 form and a copy of his/her photo ID (front & back on the same page) to include with the minor’s passport application.
  5. Payment per application
    • Routine applications: Passport book
      • Adults (age 16 and up)
        • $130.00 check or money order payable to the U.S. Department of State + $60 if expedited
        • $35.00 cash, check, debit or credit card payable to Crain Memorial Library for processing
      • Minors (under 16)
        • $100.00 check or money order payable to the U.S. Department of State + $60 if expedited
        • $35.00 cash, check, debit or credit card payable to Crain Memorial Library for processing
    • Routine applications: Passport card
      • Adults (age 16 and up)
        • $30.00 check or money order payable to the U.S. Department of State + $60 if expedited
        • $35.00 cash, check, debit or credit card payable to Crain Memorial Library for processing
      • Minors (under 16)
        • $15.00 check or money order payable to the U.S. Department of State + $60 if expedited
        • $35.00 cash, check, debit or credit card payable to Crain Memorial Library for processing

Please keep in mind that the Passport Acceptance Office hours of operation are not the same as the library’s hours of operation. See sidebar on this page for hours.

Information Regarding RENEWAL Applications

  • Only adult passports are renewable.
  • Passport acceptance facilities, including our library, do not process renewal applications, only new applications.
  • You can submit a renewal application by mail or online. Instructions are available at: https://travel.state.gov/content/travel/en/passports.html
  • The library can provide photos for a fee of $5.

You can renew by mail if your most recent passport meets all of the following requirements:

  • Can be submitted with your application and is not damaged (other than normal wear and tear)
  • Has never been reported lost or stolen
  • Was issued within the last 15 years
  • Was issued when you were age 16 or older
  • Was issued in your current name (or you can provide a document like a marriage certificate or divorce decree to show you have changed your name)

You can renew online if you meet all of the following requirements:

  • The passport you are renewing is or was valid for 10 years, and you are age 25 or older. 
  • The passport was issued over 9 years but less than 15 years from the date you plan to submit your application.
  • You are not changing your name, gender, date of birth, or place of birth.
  • You are not traveling for at least 6 weeks from the date you will submit your application. We will only offer routine service, and the time it takes to get a passport will be the same as renewing by mail.
  • You are applying for a regular (tourist) passport. You cannot renew a special issuance (diplomatic, official, service) passport online.
  • You live in the United States (either state or territory). You do not qualify to renew online if you live in a foreign country or have an Army Post Office (APO), Fleet Post Office (FPO), or Diplomatic Post Office (DPO) address.
  • You have your passport with you, and it is not damaged or mutilated, and you have not reported it as lost or stolen. Keep your most recent passport and do not mail it to us. 
  • You can pay for your passport using a credit or debit card.
  • You can upload a digital passport photo.
  • You are aware that we will cancel the passport you are renewing after you submit your application. You cannot use it for international travel.
